Contoh Prosedur Pengurusan Surat Ijin Praktek (SIP) Dokter

1.    Departemen Legal wajib memfasilitasi pengurusan SIP untuk setiap dokter yang akan praktik dan/ atau perpanjangan izin bagi dokter yang sudah praktik di fasilitas kesehatan Rumah Sakit.
2.    Setiap Dokter akan dimintai dokumen pendukung
       a.    Asli legalisir STR yang diterbitkan KKI
       b.    Surat keterangan dari Rumah Sakit bahwa dokter terkait menjalankan praktik di Rumah Sakit tersebut.
       c.    Surat rekomendasi dari perhimpunan dokter yang berada di Subang untuk dokter spesialis atau dokter gigi spesialis.
       d.    Surat rekomendasi dari Ikatan Dokter Indonesia (IDI).
        e.    Pas Foto 3x4 sebanyak 5 (lima) lembar dan 2x3 sebanyak 2 (dua) lembar.
3.    Departemen Legal bersama Formulir pengajuan SIP dan Dokumen Pendukung mengajukan permohonan kepada Kepala Dinas Penanaman Modal dan Pelayanan Terpadu Satu Pintu Kabupaten/ Kota Setempat
4.    Departemen Legal wajib membuat daftar Dokter yang melakukan praktik pada fasilitas kesehatan Rumah Sakit dan menempatkan daftar Dokter tersebut pada tempat yang mudah dilihat.
5. Perpanjangan SIP dilakukan 5 tahun sekali selama STR dokter terkait masih berlaku di fasilitas kesehatan Rumah Sakit.
